Qu’est-ce qu’une facette dentaire ?

Les facettes permettent de corriger la couleur des dents lorsque les blanchiments sont insuffisants, leur forme ( dents uséescassées, trop courtes…) et les malpositions légères (petits chevauchements, espaces interdentaires…).« Les facettes sont de fines pellicules de céramique qui se collent sur les incisives et les canines, voire les prémolaires, pour améliorer leur apparence esthétique », explique le Dr René Serfaty, chirurgien-dentiste. « On les compare souvent aux faux ongles« . Élaborée dans les années 80, la technique a gagné la pratique courante au milieu des années 2000 suite aux progrès réalisés dans les matériaux et procédés utilisés, notamment pour coller les facettes. De plus en plus attentifs à préserver l’intégrité des dents, les chirurgiens-dentistes ont aussi plébiscité les facettes comme une alternative aux couronnes qui nécessitent une taille dentaire plus importante et sont davantage réservées aux dents très abîmées. À noter : on parle d’Inlay, d’Onlay ou d’Overlay pour les dents postérieures, et de facettes pour les dents antérieures. 

À qui s’adressent-elles ?

Les facettes s’adressent aux personnes qui veulent embellir leur sourire en modifiant très légèrement leurs dents visibles ( canines, incisives, prémolaires…). Cela peut être en améliorant la couleur de la dent ou le défaut de la dent légèrement abîmée (fêlée, de travers, trop courte…). Les contre-indications sont relativement rares : « Le  bruxisme peut user et raccourcir les incisives de façon importante. Les facettes les allongeront mais elles ne tiendront pas si l’on n’augmente pas en même temps la hauteur des dents postérieures », explique le Dr Serfaty. « Un  manque d’émail important, qui peut être lié à certaines  maladies génétiques ou à la consommation excessive d’aliments et boissons acidulées, peut aussi poser problème car la colle adhère moins bien sur la dentine. Une bonne hygiène bucco-dentaire est toujours indispensable« .

Combien ça coûte ?

Leur coût est relativement important. Une facette céramique coûte entre 800 et 1 400 euros, soit l’équivalent d’une couronne céramique, et ne sont pas remboursées par la sécurité sociale. Lorsque plusieurs dents sont concernées, il peut être tentant de faire poser ses facettes à l’étranger pour diminuer leur coût de moitié. « Il est cependant difficile d’avoir de réelles garanties concernant la formation du praticien, la qualité du laboratoire, l’origine des matériaux utilisés…« , met en garde le Dr Serfaty.

Les composites, une alternative aux facettes céramique ?

Pour préserver encore davantage les tissus dentaires, notamment en cas de réparation transitoire (chez l’enfant…) ou légère (angle cassé…), il est possible de faire appel aux composites. Ces matériaux, aussi utilisés comme alternatives aux amalgames métalliques, se présentent sous la forme d’une pâte malléable qui adhère à la dent sans avoir besoin de la tailler ou très peu. La pâte est modelée jusqu’à obtenir la forme désirée, durcie à la lampe halogène, puis polie. Les composites existent en différentes teintes. Ils évitent de passer par l’étape laboratoire et sont donc moins onéreux et mieux remboursés par la sécurité sociale que la facette. Le rendu est cependant un peu moins esthétique et leur vieillissement inexorable, avec une durée de vie d’environ cinq ans.

Combien de rendez-vous faut-il prévoir ?

Les facettes peuvent être posées en deux ou trois séances seulement s’il s’agit, par exemple, de modifier uniquement la couleur des dents ou de réparer à l’identique une incisive cassée. Les changements plus importants reposent, eux, sur un véritable projet esthétique. Ils nécessitent davantage de rendez-vous pour les essais et, dans l’idéal, la réalisation d’un masque qui permet de simuler le résultat final en bouche.

  • « Lors du premier rendez-vous, la personne m’explique pourquoi ses dents ne lui plaisent pas. Je les examine, je prends des photos, des radiographies et les empreintes qui me permettront d’obtenir un moulage en plâtre sur lequel travailler« , explique le Dr Serfaty. Le spécialiste prépare ensuite la prochaine consultation en ajoutant de la cire au moulage :  » Je me base sur les souhaits du patient et certains critères esthétiques (alignement, hauteur des dents, courbure du sourire…). Quand le résultat semble convenir, je fais réaliser une gouttière » ;
  • Au deuxième rendez-vous, la gouttière est remplie de résine et placée dans la bouche du patient« La résine recouvre les dents et durcit pour former un « masque » qui, une fois la gouttière retirée, permet d’avoir une bonne idée du résultat potentiel. Nous rediscutons de la forme, de la couleur des dents… Je prends de nouvelles photos et laisse éventuellement le masque à la personne pour qu’elle puisse réfléchir au projet et demander l’avis de ses proches ».

Comment sont-elles posées ?

Le praticien prend ensuite de nouvelles empreintes des dents préparées puis pose des facettes provisoires pour les protéger et les rendre présentables en attendant les facettes définitives. Un essai intermédiaire est souvent programmé avant cette dernière étape. « La pose a lieu sous anesthésie locale, avec mise en place d’un champ opératoire. C’est une séance particulièrement longue et délicate, où il est essentiel de respecter un protocole précis« , remarque le Dr Serfaty qui compte une matinée pour six facettes.

Quelle est la durée de vie d’une facette dentaire ?

« Le succès des facettes, tant pour l’aspect esthétique que pour leur durée de vie, est « opérateur-dépendant », lié à l’expérience du chirurgien-dentiste et de son prothésiste, remarque le Dr Serfaty. « Si l’indication est bien posée et le collage correctement réalisé, plus de 90 % des facettes sont encore présentes et esthétiques douze ans plus tard ». Les échecs sont le plus souvent représentés par des fissures ou fractures de la céramique, parfois par des infiltrations qui modifient la couleur des facettes et augmentent le risque de carie. Les décollements sont rares.

Comment entretenir une facette dentaire ?

Les facettes s’intègrent naturellement au reste de la dentition et sont bien acceptées par la gencive qui retrouve sa place en quelques jours. Elles ne nécessitent aucun entretien particulier mise à part une hygiène bucco-dentaire impeccable (utilisation de  brossettes et fil dentaire,  détartrages réguliers…) et le port d’une gouttière de protection nocturne pour les personnes sujettes au bruxisme.

Peut-on enlever une facette dentaire ?

Pour ne pas se casser, les facettes doivent avoir une hauteur et une épaisseur suffisantesce qui impose souvent de tailler les dents. Dans tous les cas, l’émail est partiellement éliminé afin de permettre à la colle d’adhérer. Cette étape rend le traitement irréversible, obligeant à porter des facettes à vie.

Brossez-vous les dents 2 fois par jour pendant 2 minutes

La plaque dentaire, dépôt blanchâtre où prolifèrent les bactéries, sont à l’origine des caries et des inflammations de la gencive. Comme elle se reconstitue en quelques heures, il est indispensable de l’éliminer par un brossage au minimum biquotidien. Dans l’idéal, il faudrait se brosser les dents après chaque repas, mais comme ce n’est pas toujours possible au quotidien, les dentistes conseillent de se brosser les dents deux fois par jour pendant deux minutes. 

Choisissez une brosse à dents souple

Pour un brossage efficace et agréable, il est important de bien choisir sa brosse à dents. Idéalement, ses poils doivent être souples. En effet, une brosse à dent dure risque de blesser les gencives et d’abîmer l’émail des dents, surtout chez les enfants ! Une brosse à dents usée n’est pas non plus efficace. Les brosses à dents électriques peuvent être avantageusement utilisées, à condition d’en changer régulièrement l’extrémité.

Changez votre brosse à dents tous les 3 mois

Il est nécessaire de changer sa brosse à dents régulièrement, idéalement tous les trois mois. N’attendez pas qu’elle ne ressemble plus à rien, car lorsque les poils sont repliés, le brossage n’est plus efficace. De même, si vous avez été malade, veillez à jeter votre brosse à dents après guérison pour se débarrasser  des microbes qui restent dans les poils.

Utilisez la bonne technique de brossage

Posez la brosse à dents inclinée à 45° à cheval sur les dents et la gencive et brossez en même temps les dents et la gencive, haut et bas de la mâchoire séparément, en allant de la gencive à la dent, avec un mouvement de rotation. Idéalement, le brossage devrait durer 3 minutes, mais s’il dure 2 minutes, c’est convenable !

Complétez le brossage avec d’autres accessoires (fil dentaire, jet d’eau..)

L’utilisation d’un fil dentaire en complément du brossage permet d’éliminer la plaque dentaire sur les faces latérales des dents. Pour les personnes ayant les dents déchaussées ou des prothèses qui laissent des espaces inaccessibles à la brosse, il est important d’utiliser des bâtonnets ou des brossettes inter-dentaires pour éliminer la plaque dentaire à la base des dents et stimuler la gencive. Les appareils à jet d’eau sont également utiles dans ces cas pour éliminer les débris alimentaires et masser les gencives.

Allez chez le dentiste au minimum une fois par an

Une visite annuelle chez le dentiste permet de détecter à temps des caries débutantes et sera l’occasion de pratiquer un détartrage pour éliminer la plaque dentaire (deux séances de détartrage sont remboursées par la sécurité sociale !). Si la plaque dentaire n’est pas détectée à temps, elle se développe en profondeur et forme une poche entre la base de la dent et la gencive, qui risque de provoquer, à la longue, un déchaussement des dents. Sans parler des caries qui peuvent se développer sans que vous le sentiez !

Envisagez la brosse à dents électrique

Une brosse à dents électrique a de nombreux atouts, et beaucoup ne peuvent plus s’en passer ! Sa petite tête se glisse partout, et les mouvements que nous conseillent les dentistes se font tout seuls. Leur efficacité sur la plaque dentaire est supérieure à un brossage manuel. De même, elle diminue  le risque de saignements.

Veillez à l’hygiène dentaire de vos enfants

Dès le plus jeune âge, il faut être attentif à la bonne hygiène dentaire des tout-petits. Les biberons d’eau sucrée ou de lait laissés au petit enfant le soir dans son lit font des ravages, en laissant se développer plaques dentaires et caries précoces. De même, les bonbons sont à proscrire le soir. Chaque fois que possible, il faut préférer des bonbons ou chewing-gum sans sucre. Des apports supplémentaires de fluor chez la mère, pendant la grossesse, et dans l’enfance peuvent être conseillés par le médecin pour renforcer les dents.

Apprenez à votre enfant à se brosser les dents

Il est indispensable de faire connaître à l’enfant l’usage de la brosse à dents. Si l’on conseille de masser régulièrement les gencives avant l’apparition des dents, on peut commencer à brosser sans dentifrice la première dent de lait. Entre 0 et 3 ans, le brossage doit être réalisé par un adulte. Puis, entre 3 et 6 ans, il peut être assisté par un adulte, en fonction des capacités de l’enfant. Pas toujours facile de faire comprendre ce geste, mais cela peut être un petit jeu qu’il apprendra peu à peu à faire seul dans de grands mouvements désordonnés…

Adolescents, faites attention à vos dents

À l’adolescence, le risque de caries dentaire est particulièrement élevé. Dans 80 % cas, ces caries se développent dans les sillons des molaires. En coulant un film de résine synthétique fluide sur les sillons, il est possible de prévenir ces caries. Ce traitement peut être réalisé dès la sortie des premières molaires. Il est remboursé par la sécurité sociale jusqu’à l’âge de 14 ans.
